Job Description: Healthcare Coordinator We are seeking a compassionate and dedicated caregiver to join our team at Meta Care, Inc. as a Healthcare Coordinator for Member Support. In this role, you will be responsible for assisting and supporting members with their coordination of healthcare needs, ensuring their safety, well-being, and overall health.
Responsibilities: - Schedule and coordinate healthcare appointments for Member members, including medical, dental, and specialist visits.
- Encourage good daily nutrition and dietary requirements for Member members, ensuring they follow appropriate dietary guidelines.
- Provide emotional support and be a listening ear for Member members, offering comfort and understanding during challenging times.
- Collaborate with healthcare providers to ensure seamless communication and coordination of care for Member members.
- Monitor and assess the overall health and well-being of Member members, reporting any concerns or changes to the appropriate healthcare professionals.
- Serve as a liaison between Member members and healthcare providers, ensuring effective communication and understanding of healthcare instructions.
- Maintain accurate and up-to-date records of Member members' healthcare needs, appointments, and progress.
Requirements: - Valid Licensed Practice Nurse (LPN) license; or Certified Nursing Aid (CNA) or Medical Assistant (MA) license in Texas
- Minimum of [3] years of experience preferably in a healthcare coordination or support role.
- Excellent communication and interpersonal skills, with the ability to listen and provide emotional support.
- Strong organizational skills and attention to detail, with the ability to effectively schedule appointments and maintain accurate records.
- Humble and personable demeanor, with a genuine desire to assist and support Member members.
- Ability to work independently and collaborate effectively with healthcare providers.
- Flexibility to work onsite at least two days a week for [6] hours total (3 hours per day/2 days), with the potential for this position to grow into a part-time or full-time role.
