A community services organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff upstate New York as a Nurse Case Manager. In this role, the Nurse Case Manager will be responsible for developing plan of care and coordinating the delivery of healthcare services for persons receiving services in an assigned program unit.

• Schedule: Monday to Friday
• Hours: 9am to 5pm
• Patient Population: Adults

Responsibilities:

• Coordinate the delivery of health care services for adults with disabilities residing in residential programs
• Be responsible for performing nursing assessments and diagnoses
• Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.
• Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated. Maintains health care profiles.
• Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.
• Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers.
• Completes and maintain all required documentation on the approved electronic health record system.
• Charts health care progress as indicated by acute illnesses and/or complex ongoing medical problems.
• Maintains all medical and nursing standards as set forth in agency policy, OPWDD regulations, and NYS Nurse Practice Act
• Completes relevant medical documents, MIPS and other required consent packets, surrogate packets, writing incident reports, death reports, assisting in developing policies, procedures and forms; and establishing working systems.
• Performs all other duties as assigned.

Qualifications:

• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license
• Experience working with persons with Developmental Disabilities
• Solid assessment, clinical, and documentation skills
• Patient oriented
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized

Desired Skills:

• Bachelor's Degree in Nursing
