The Social Work Assistant supports and assists the Social Workers in the task of meeting the psychosocial, financial and well being of the patients that are served by the Kaweah Health. The Social Work Assistant also lends support to the mandated components of Joint Commission requirements in Advance Health Care Directives.

QUALIFICATIONS

License /Certification
Required: Kaweah Health approved crisis intervention training within 60 days of hire/transfer.

Education
Required: Associate's degree or equivalent experience in a medical setting working with an interdisciplinary health care team.

Experience
Required: Medical terminology experience

Knowledge/Skills/Abilities
Excellent communication, interpersonal, and writing skills.
Excellent organizational and problem solving skills.
Able to work independently as well as within the team.

Testing
During the interview applicants will be asked to prepare a written document based on facts provided.

JOB RESPONSIBILITIES

Essential

Assists in financial counseling; linking patients and their families to appropriate resources in collaboration with the Social Worker and RN Case Manager.

Participates in Interdisciplinary Team meetings and assists in discharge planning as needed, or as directed by the Social Worker or RN Case Manager.

Facilitates charity assistance for needed medication or clinical visits for qualified Residents as authorized by PFS Manager or Director of Case Management.

Responds to Advance Health Care Directive orders and follows up with patient and families to ensure compliance with Joint Commission requirements.

Assesses and coordinates transportation via taxi and/or bus to facilitate timely discharge.

Assesses and coordinates assistance with obtainment of essential medications for the indigent patient population.

Coordinates with Case Management, Social Workers and Financial Counselors to ensure that patients discharge needs are being met.

MENTAL HEALTH ALSO RESPONSIBLE FOR:

Coordinates placements for patients (IMD. Board and Care, SNF). Performs Discharge Planning as directed by the Social Worker. Creates letters for physicians and families related to insurance, immigrant status, jury duty or other as indicated or as directed by the Social Worker. Coordinates Discharge Packets for appropriate follow up with outside Mental Health Clinics.

SUBACUTE/TCS ALSO RESPONSIBLE FOR:

Provides patient/family/legal guardian with the Resident Admission Agreement and completes all paperwork associated with the Agreement for patients admitted to Sub-Acute Care Services and Transitional Care Services. Completes all assigned sections into the MDS (Minimal Data Set) and Care Plans for patient mood and psychosocial problems. Keeps Care Plans up to date and pertinent to the patient's social needs/problems. Creates letters for physicians and families related to insurance, immigrant status, jury duty or other as indicated or as directed by the Social Worker or RN Case Manager. Coordinates with the benefits department and financial counselors to ensure accurate financial data is maintained for all Residents including tracking of Medicare days. Assists Residents with obtaining and completing necessary financial forms. Assists Residents with personal finances as indicated. Presents and explains Skilled Nursing Facility Advanced Beneficiary Notices (ABN) and Expedited Notices in conjunction with Case Management and Nursing. Provides support to families and residents, participates in family conferences, takes social histories, and receives and resolves grievances acting as a Resident advocate. Meets with residents weekly for support and to resolve any identified issues. Refers complex cases to Social Worker or RN Case Manager as appropriate. Conducts meetings and keeps records for formal and informal Patient and Family Resident Council as required by DHS. Documents histories and weekly Resident meetings in the medical record electronically. Coordinates treansportation for patients requiring trips to docotor's offices or special procedures performed outside the facility as well as social /entertainment trips (when authorized by the Administrator). Accompanies Residents for appointments and trips as indicated. Assists Residents with needed errands.

Additional

Regularly attends staff meetings for South Campus, Patient and Family Services and Case Management.

Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area.

Performs other duties as assigned.

Pay Range
$21.61 -$32.42
